Patient Access to their GP record
Changes to the GP Contract require all practices to offer and promote the facility for patients to view on-line, export or print any summary information from their records relating to medications, allergies, adverse reactions and additional data that the doctor has agreed the patient may have access.
This is an extension to the service we already provide which allows patients to make appointments, cancel appointments and order repeat medications.
In preparation for this we would encourage all patients to register for ‘On-line Access’. You will need to complete a form at reception and provide photo identification and a recent utility bill (within 3 months). We will then process this and give you log-in details.
Keston Medical Practice intend to have this facility available to patients by 31st March 2015.
A Summary Care Record (SCR) is available for all patients by default. This allows information about medicines you are taking, allergies you suffer from, and any bad reactions to medicines that you have had to be available to healthcare staff in healthcare settings outside Keston Medical Practice (with your consent, if you are able to give it). Your SCR is automatically updated at least daily to ensure the information is up-to-date.
You may opt out of the SCR – to do this you will need to complete a form, available at reception.
Moving House/Changing Doctor
Electronic medical records can be transferred between Keston Medical Practice and practices that use the same computer system. This means that once the registration process at your new practice is complete your medical record will be available to them. This transfer facility is called GP2GP.
